NOTE: Use your Tab or Enter keys to navigate through the form fields. Many of the fields have a limit to the number of printed characters they will accept.

Please note that each program file is available in 2 version types. The EXE version is a self extracting executable compressed file and the ZIP version is a zip compressed file. The zip format will require an external program such as Pkzip or WinZip to uncompress the file. Because some internet firewalls may be restricting the download of executable (.exe) extension files, the zip format is now being made available.

The Wisconsin DNR Well Forms Program allows you to build your own electronic database of well information collected from eight (8) DNR well related forms:

Monitoring Well Construction Form 4400-113A , Monitoring Well Development Form 4400-113B, Well/Drillhole/Borehole Abandonment Form 3300-5 , Groundwater Monitoring Well Information Form 4400-89, Soil Boring Log Information Form 4400-122 and 122A , Groundwater Monitoring Inventory Form 3300-67, First Water Quality Report Form 3300-77, and Drinking Water Well Construction Form 3300-77A.

The program allows you to enter and edit information, print copies, export, and import the data in Microsoft Access 97 database format. Copies of this program can share data using the data Export/Import feature. The program can be installed under Windows 95, 98, ME, NT or XP. This is a self extracting executable file. Please download and install only the version update (below) if you already have a full program version before 4.1.3.

Version Upgrade to 4.1.3 Only.

Update created on June 5, 2003. You must have a FULL version (above) already installed. This update is available in a self extracting executable or zip file version. These files replace version updates before version 4.1.3. The current program version is 4.1.3, located on the Main program screen in the upper center portion of the window. If you do not have program version 4.1.3, you should download this version update. These files will replace/overwrite the existing program files default program directory. If the program was installed in a directory other than the default, please make sure you direct these update files to that new directory location during the unzipping process.

Important Critical Information: This version will replace your existing database file with a new one (if you have NOT previously upgraded to version 4.0 or higher), reload any existing data, and create an automatic backup of the old database. This process will run when you access the main program menu screen. If you do not complete the database file update, the process may try and run each time you try to access the Drinking Water Well Construction form, First Water Quality Test form, Soil Boring Log form, Groundwater Monitoring Inventory form or the Well/Drillhole/Borehole Abandonment form .

This file contains the definition of the output file format for monitoring well construction and development information that allows you to create a text file suitable to be uploaded by the DNR from your own software application, spreadsheet, database, etc. Users of this format can email their submittal to GEMS@dnr.state.wi.us placing Monitoring Well Construction and Development Form in the subject line.
Variance for Water Supply Well Setback to Landfill Application 69K 05-02 Application for Landfill Variance: Form 3300-209 This form is required by the DNR Drinking Water and Groundwater Program to obtain a variance under NR 812.43 to reduce the 1,200 foot setback to a water supply well.
