This form is intended for use by applicants who have submitted an approved Pre-proposal Letter of Interest prior to the deadline for this RFP. If you did not submit a Pre-proposal and are interested in applying for LIP funding, please feel free to contact LIP staff in anticipation of the next cycle.

Completed proposals must be RECEIVED by the Department by January 16, 2009. Online submission is strongly recommended but other arrangements can be made through the LIP coordinator prior to the submission deadline. You may access a PDF version [PDF 148KB] of the proposal as well.
